New Membership

To apply for membership, please prepare the following documents and send them to the ICICH website administrator, Angela Labrador, alabrador@jhu.edu, who will share your materials with the bureau:

1. ICICH membership application form
   • ICICH membership application form (English-French)
   • ICICH membership application form (Spanish)
2. Your current Curriculum Vitae (CV)
3. Reference letter: either a reference letter from your National Committee or two letters from ICICH members
4. Proposed activities: a brief note outlining the activities you intend to pursue as an ICICH member over the next five years. 

You will need Adobe Acrobat, Acrobat Reader, or comparable PDF software to fill in the form on your computer.
